I have two 828 MkIIs which are both firmware 1.00. These have caused me many problems over the years, many dropouts and odd behaviours- I use them both with Macs and PCs. I found out from a very helpful person how to update to firmware 1.01 without replacing any eprom chips or suchlike. Im not sure that this will iron out any of your problems, since I know 1.01 owners, hell even 828 Mk3 owners get their fair share of oddness too. But hey, I've torn out out what little hair I have left over these units and will try anything.
(You need to do this on a Mac-if you havent got one, try and borrow one, please also note I do not have the expertise to tell you how to reinstall firmware 1.00 should you wish to do so)
So here is how:
1.Download the below linked file

2.Unzip the folder, it contains two files.
3.Power on 828 MkII with 'cursor' button held down which switches the unit into firmware upgrade mode.
4.Open the Terminal (under Utilities in the Applications folder).
5.Drag the 'DownloadFlash' file into the terminal.
6.Type [-w][space][-f][space] then drag '828Mk2_1.01.bin' file onto terminal.
7.Press enter/return. It takes a minute or so.
8.Power cycle the 828. Bob's your uncle.
Below is what my terminal screen looked like at the end of the process:

Dear Dr. Zygote, My MOTU 828MK2 has been entirely reliable for years, except for rare squealing or the occasional freeze...however, this morning, I am suddenly faced with a HUGE LOUD HUM emanating from all audio outs, even in standalone mode, no matter how many times I reboot, reset to defaults, reinstall drivers, let it cool down, take it for walk, etc...so I was enthused to see your method of reinstalling firmware, since often it will reboot in "firmware upgrade" mode even when I do NOT hold down cursor key...but it appears that the firmware link you provided is broken, and I have exhausted all avenues of finding the firmware driver, you were my last hope. This unbearable noise issue is hitting just as I am completing two important projects, of course! I appreciate your help in reposting a working link, or any advice you or anyone on this board could offer. Thank you kindly, codigi

You're probably violating MOTU's license agreement by redistributing their software without authorization.

Well, I finally got this to work, using my daughter's G4 Powerbook that would actually see my 828MkII. Unfortunately, my MBP with FW 800 still cannot see the 828. Major bummer. I've really been hoping this would be the fix I need for that to happen. Oh well, putting my 8pre first in the chain still works, so that's how we'll fly.
