Not totally fail, but there are many of us who have had it fail to be recognized on boot for unexplainable reasons. Mine hasn't done that since way back in the early days, and it usually returned on reboot (knock on wood).
Since you've moved slots, and you didn't mess with anything when it stopped working(?), it sounds a bit worse, but don't lose hope.
The failure to "see" the i/o box, and the port swapping thing, is probably different (I've had that one several times too). I think if the card works, your machine should show some sign of it somewhere, even if there are no drivers or interface boxes. I'd at least make sure that it does the same thing in a totally
different computer, and with a clean (or different) driver install. If you're "lucky", maybe something screwed up slightly with the PCI bus on the host itself (if you could call that "luck"). Also, see if you can clean the card's contacts with an appropriate (safe) cleaner/solvent, maybe blow the motherboard's PCI slots out with some air, and make sure there isn't anything "broken looking" on the card itself (like those wonderful "last minute" jumper wires on mine).
You seem like you've probably tried that sort of stuff. Worst case, maybe you can just pay for one of the card upgrade deals and swap it back to them, rather than having to buy something you already owned and didn't
personally destroy. I doubt the previous PCI card versions are worth much to them either way, so it probably ends up in a pile (or dumpster), or worse yet, goes back to an unfortunate someone like yourself, who's paid to have their's repaired (guess they test those first anyway).
