Got an issue with MIDI set up and the 828 mk3.
I've just had to re wire the 'studio' and now the MIDI side of the 828 keeps giving me a new icon every time I boot, greying the old out and breaking the connection to the keyboard. So I get 828 mk3 then 828 mk3 #2 and 828 mk3 #3 and so on everytime I boot up. At first I though it was the fact that I originally had this wired direct from the mac and then after the change round I wired it of a FW hub. but going back to a direct FW connection made no difference.
I have tried deleting the prefs folder in the user library. To no effect. I also removed the user library folder and reinstalled the driver. Again no effect. I can't verify permissions as the user library is on a MacProRAID 5 card and it's not an option in disk utilities.
Obviously I don't want to go back to wiping the OS and user library again as I have other software on and it's a pain to reinstall and go through the setting up of everything.
Any advice would be appreciated.

I had this issue also. It went away. As to why, I can tell you I did these things. (Unfortunately, I can't tell you which worked as it has been a little while.)
I reinstalled the USB MIDI drivers.
I reinstalled the FireWire audio drivers.
I reinstalled the FireWire audio drivers "properly". (There is a tech note on your exact issue...the crux is that, according to the tech note, the audio driver - which I guess also installs MIDI components - was installed while the 828s were powered up and that is not the way to do that. Oddly enough, I don't remember this as the problem solver, but I definitely followed the tech note's advice.)
I completely powered down the system and unplugged the FireWire cables, reattached them, powered on the 828s completely, then booted the machine.
I sacrificed a chicken and spread its entrails in front of my racks, left an offering, and burned incense.....(ok. Maybe not. But I was about to...)
I did these things a couple of times in different orders as I was also having a problem with getting my 828s (3 mk2s and one hybrid) all recognized in MAS Setup. Spmetimes the audio side was right and the MIDI was screwy and vice versa. Eventually everything worked although occasionally one of the 828s will grey out (and usually the same one).
Sorry I can't be more scientific about the steps, but this occurred over a couple of days and I was getting a bit desperate to fix it.

Try waiting until after you log on to power up the 828 units. Don't know if we have the same problem, but it's a known issue on Mountain Lion and some systems. If that doesn't help, you could also try deleting your 'By Host' MIDI preferences.
