To follow, here are my thoughts and discoveries.
First here is my set up
Dual Gig G4
OS 10.3.6 (good so far)
1,5 gig RAM
DP 4.12
Virtual synths
3 X MTP AV
2408
2 X 308
4 PC based samplers running EWQLO Platinum
1 PC based sampler running GigaStudio 160
1 Mac (9600) sampler doing old samplecell
But here is an interesting thing. I recently acquired MIDI over Lan and sucessfully installed it on the G4 and the 4 remote pcs doing the orchestra. (I'm not using much of the other two samplers these days) I got the MIDI stuck notes even there... So I think that Tony may have sometinhg going with the audio clock... Also I had a friend composer with the very same 4 pcs orchestra setup but using the three MTP AV routed to a usb hub with the same MIDI stuck notes and we discovered that the problem was... THE HUB! So everyone, use a usb 1.0 hub, not a 2.0. It causes the MIDI signal to be clogged up for some strange reason. (But that doesn't explain why I still get, from time to time and not so often as some of you, stuck notes.) I'm going to use a nanosync rosendahl soon to clock up everyone and see if it works.

I'm not seeing a definite pattern yet, but here are two variables which may be figuring into the mix: OS X version and USB hubs.
I'm using OSX version 10.3.4. I've deliberately avoided upgrading, because everything has been so stable on my system. Now I'm absolutely paraonoid to upgrade. While I do have USB hubs in use, the MIDI Time Piece is not connected to one. It goes directly into one of the computer's two USB ports.
I use my computer for a variety of things, so no dedicated setup here. Oh, one other variable: I did the permissions repair thing before and after install of DP 4.5. There is no way for me to tell if that has anything to do with anything, but it's a variable, so there it is.
Anyway, I still have had no hanging MIDI notes. Everything is working as expected here, and I don't even get the dreaded "crash on quit." Still, you know how things are. I could be working in a file that suddenly develops these problems. If so, I'll post here about it.
I hope we can find something in common with everyone who is getting the stuck MIDI notes. I want to get to the bottom of it, because if it were to happen to me, it would mean DP is no longer suitable for live performance. THAT would be VERY serious.

For all the folks having stuck notes on large audio-MIDI seq with DP 4.5, if you want to, hopefully, smile again, TRY TO USE MEDIUM PRIORITY ON HARWARE SETUP, COUPLED WITH 512K BUFFER.
I had stuck notes, tried different settings, and after several test, this last MOTU advice fix my pita. Be sure to have the latest MOTU-USB and AUDIO drivers.

Dan, I did try your medium priority with dp 4.5 but as I wrote too before, it did not change a thing for me. (I wish it did though)
I also wanted to make my point clear about usb hubs: I meant to say do NOT hook the MTP AV to ANY usb 2.0 ports on ANY device (Maybe it works with the G5, i don't know). As far as i understand it, the MTP AV usb protocol (usb 1.0) is conflicting when hooked to usb 2.0 protocol.

I noticed that my mtp a/v isn't connected directly to the computer but through a hub...I'm going to try bypassing the hub and going right into the comp, hopefully, my MIDI sequences will run smoother now.Originally posted by Shooshie:

Well I don't have quite the problems that have been described above, but I have noticed since upgrading to DP4.5 that I get hanging MIDI notes when using Quicktime instruments in both DP and in Band In A Box. The only way to unstick is to shut the offending program. DP behaves fine when i use hardware instruments or VSTs; my MTP AV is also connected directly to my G5, I'm running 10.3.6 and my final 'bug' is that my BOSS DR-5 hangs with a Chk Sum error whenever I hit Play in DP. I'm inclined to fault Apple and the Audio MIDI Setup - DR-5 does not have its own .middev and its unclear to me how Quicktime relates to the AMS!

Speaking of hubs, I have two cheap 1 to 4 hubs, non-powered off the back of the display. They go to two 128 MIDI express and one micro express. I need both beause I also have iloks etc. hanging off the hubs.
I get occassional stuck notes on the PC giga's but I have an issue were every once and while one or the other of the 128's will not show up. I have to unplug and replug both or all three several times to get it back.
I'm convinced that it's a hub issue.
What's the best hub to use? Anyone have any ideas?

I've had the same problem with my 3 MTP AV connected to a McAlly 3 port usb (1.0) hub. Did the same as you, unplugged than plugged again. This may be due to the non-powered hub though. I suggest you give it AC to see what it does. Usb is not very reliable anyway, and we're stuck with it! For my part, I got MIDI over lan CP which corrected that problem. Check it out, it's cheap enough and runs on both pcs and macs.

1. I'm using the MIDI Express XT not the MTP - but might not be a difference in this.

I meant MIDI Xpress and it should not make any difference.
Again, this may (I hope) or may not (...) be a clocking problem since all of us here have had more or less the same problem on;
1. Different versions on DP (back to 2.x ...)
2. Different OS (Back in the old golden days of OS 8 and the lattter)
3. Different machines with different RAM configurations, plugs, adb/serial connections over usb, etc...
I am about to hook a external master clock to see if it makes any difference even though my system has been stable for a while now.
