Mic distorting

Probably something basic, but any ideas what might be causing mics to distort (or sometimes sounds like a small click or buzz). Levels all look good both on the preamp itself and in the Audio Monitor in DP but I'm hearing the unwanted noises. To get it to stop, I've had to lower both the gain and the output to very low levels, at which point the level into DP is pretty low. So I can crank up the channel level on my Motu828 to compensate, but I feel like I should be able to figure this out just using the preamp/compressor. FYI, I've noticed it mostly (or possibly only) in case where phantom power is needed.
