Our next step will be to start a recording when it starts overloading the channel and see if audio is actually being created (maybe the LEDs are just crapped and everything is fine). Apparently, the ROM chip has failed.
We went to record some audio, last night, and the unit was sitting on the firmware upgrade screen.
A call to Motu support, this morning, concluded that loosing the routing was indicative to the failing ROM.

When I've seen it happen, inputs still worked, but outputs didn't (no routing). We use it, strictly, for recording and monitoring. Well... We did a factory reset and it is still happening. A couple other points... He was using power over firewire, when it started happening. He switched to mains power and the problem persists.
