I'm Only Getting Left Channel Headphone Output!

Please help..... I'm sure there's an easy solution here. I'm only getting audio in my left headphone channel. If I pan in the mixer, the audio drops to nothing on the left side. How do I get full stereo on the headphone outs??? Please help with troubleshooting ideas!

Make sure that you don't have a mono source recorded on a stereo track.....

Are other apps sending stereo to the headphone? Could it be a bad jack connection?
