Usually, I mic the kit as follows:

Audio Technica 4047sv: Kick (approx 1-2' out front outside of shell)
Shure SM57: Top of Snare
Oktava mk012 (pair): Overheads in Glyn Johns pattern
Sennheiser e609: Rack Tom
AKG D112: Floor Tom

I just recorded drums for 24 songs between April and June in a 200 yr old barn. I had problems with my Oktavas picking up RF interference, so I was forced to do the following:

Audio Technica 4047sv: Kick (approx 1-2' out front outside of shell)
Shure SM57: Top of Snare
AKG 414 B/ULS: Mono Overhead on Omni
Microtech-Geffel UM70: Mono Room/Front of kit on Omni
Sennheiser e609: Rack Tom
AKG D112: Floor Tom

This turned out really great and was a wonderful surprise. I love my Russian Oktavas but the 414/UM70 was much better. I wish that the UM70 was mine. Killer mic, a poor man's U87 for sure. Sounds amazing on vocals, Vintage Fender amps, percussion, acoustic guitar, etc.

has anyone tried the Kickball from Blue? i've been using the akg 112 on kick drum but its owner will need it back soon. i'm considering the Kickball instead of the 112. any thoughts?

Once, I tried a kick drum mic, a Sennheiser e602 on my floor tom. And it's stayed there ever since. It sounds huge! Try it!
