I've done a test, recording both audio and MIDI data simultaneously from my Yamaha CP4 digital piano.
The MIDI data arrives about 20 ticks after the audio data.
How can I fix this?

did you try from the menubar: Setup->Configure Audio->System->Fine-tune Audio I/O Timing ??

This.
Curious... did you test the timing at different buffer sizes? Was it consistent?

After giving up in frustration, I've come back to this issue again.
I don't seem to get much difference in lag with different buffer sizes. I don't feel confident in adjusting
Setup->Configure Audio->System->Fine-tune Audio I/O Timing
I've noticed since using my Focusrite Clarett interface that I cannot monitor as Direct Hardware Playthrough. Is that an issue?
Getting really paranoid about my time playing!

Check this settings:
Preferences and Settings - Play And Record - MIDI Options -
Patch Thru in background (try On or Off)
Sync Recorded MIDI to Patch Thru (try On or Off)
However, I promise nothing
