Forgive me if you already know this but the volume/swell on the B4II is mapped to cc11-Expression by default. I know it's possible to change the controller assignments within B4II but on the off chance you are unaware of the cc11 default that could be your problem.

I have done this internally. I use a USB headset when Skyping. The headset shows up as an Audio hardware option in DP as well as Core Audio. As I recall I put all of my audio sources on a bus and then split that bus between two auxes one going to the Headset output and the other to your regular outs...

I've seen extra notes (two to three of the same notes playing at the same time) in two different scenarios. One is if you have a keyboard that can create different zones and more than one is turned on and the zones overlap each other. Where they overlap you will get two notes played at the same time...

here are some things DP doesn´t have... 1. no MIDI regions in the SE - it´s so cumbersome to copy MIDI and it´s so confusing to get an overview of the arrangement. it´s so much easier to see what is the musical context in Logic, Cubase, Nuendo, Sonar, Live, Samplitude.... I don´t know any other DAW...

Absynth4 and PPC No GO
I know I'm not the only one to report this but Absynth 4 comes up with a blank white window in DP6 under 10.5.4 with aDual2x2 PPC. Sad to say

I'm running DP 5.13 on a Powerbook G4 1.67ghz with 2 GB RAM, 828 MK 2, OSX 10.5.3. It's working fine, however, I'm sorry I updated to Leopard on my Powerbook. It's much slower than 10.4.11 was, so if you haven't moved to Leopard on your Powerbook, my recommendation is to stick with Tiger. IMO, Leop...
