Motu mk3 UltraLite - extremely quiet output

The output of my Motu Mk3 UltraLite is extremely quiet. I have tried multiple factory resets, swapping cables + different outs, and still nothing.

As can be seen in the attached picture, the signal in as well as out is peaking (due to me cranking the trim on the input), and yet the signal received by my UA Volt is barely audible.

If any more information is needed, I would love to provide.
