8 Pre OSX Low Level Distortion

I'm running an 8 pre on 10.11.6.

On play back (this can be through any audio program) I'm hearing a duplicate signal of the main mix that is really low but distorted heavily.

If I turn the volume all the way down to the lowest setting this is when you can hear it as the main mix is almost off but the distorted low level version is still there (this doesn't change when I change the main mix volume.

I'm basically getting a duplicate signal of the main mix that is heavily distorted but very low in the mix that isn't affected by me altering the main volume.

I've tried updating the drivers and completely uninstalling and reinstalling but can't figure out whats going on? Any help would be gladly excepted.
