I emailed MOTU support. They say they don't support this unit anymore. Say they have no spare parts. I ordered a used one and just Got it in and hooked up today. Works fine. I make an attempt to fix my other one my self if I can't find someone else to tackle it. MOTU won't provide schematics. Bummer...

It will work fine sometimes. At times it will not be recognized after power up. It sits down after several minutes whin it works at all. Sometimes it might run for an hour or more. Makes me think it is a hardware issue with the unit. Could be the internal power supply I guess.

Thanks, James. The app that you mentioned was the problem. I had to download an updated version of MOTU Audio after which all the inputs and outputs showed up.

The issue: Audio MIDI Setup sees the PCI-424, but it only shows 8 ins and 8 outs. I have a mac pro 8 core with latest OS; Sierra.
